If someone asks if the cat has got your tongue, they want to know why you are not speaking when they think you should.
EXAMPLE:
TOM: Come on, Loree! Tell me what you think about my new shirt.
LOREE: Well, uh . . .
TOM: Isn't it great?
LOREE: I, uh . . .
TOM: What's the matter? Cat got your tongue?
LOREE: Well, if you must know, I'm keeping quiet because I don't like it so much.
